DMH Home Health Agency
The Delta Memorial Hospital Home Health Agency provides skilled nursing, physical therapy, certified nurse aide services,occupational therapy and speech therapy.
All Home Health Services are provided in the comfort of the patient's home. Referrals to Home Health are accepted from your physician, a discharge planner, or by family request. Each patient is individually evaluated by a registered nurse, and a plan of care is developed by Home Health in conjunction with the patient and their physician. In most cases home care services can be covered through Medicare, Medicaid or Private Insurance however private pay can also be accepted. Medicare patients must be homebound to qualify for health services.
Our Home Health Agency is there for you 24 hours a day, seven days a week. Our on call Home Health Nurse can be reached by calling Delta Memorial Hospital at 382-4303.
Our office hours are 8:00 a.m. to 4:30 p.m., Monday through Friday. The Delta Memorial Hospital Home Health Agency is located in the Delta Health Services Clinic Building on South 65 Highway next door to Delta Memorial Hospital.
