Delta Memorial Hospital Employee Benefits

1. Term life insurance which equals 1.5 x earnings up
to $100,000.00 is provided to full time employees by
Delta Memorial Hospital at no cost to employee.

2. Delta Memorial Hospital provides a 401K
retirement after one year and a minimum of
1000 clock hours.

3. Enrollment for benefits must be completed during
the first 30 days of employment.

4. Employees classified as full-time and part-time
receive paid time off (PTO), which combines vacation,
holidays and sick time. PTO time accrues immediately
upon employment, but cannot be used until after 90 day
probationary employment period is completed. Our current
PTO plan provides a new employee 0.7% per daily hours
worked of PTO for each biweekly pay period.

5. PRN employees receive no benefits.

Our current health insurance plan includes Blue Cross & Blue Shield Medical and Dental plans. Listed below are Medical and Dental rates per biweekly pay period.

Employee & Spouse - 24.32

Delta Memorial also offers various other optional
supplemental insurance plans available by payroll
deduction.
